Required Documents & Supporting Documentation
To enroll in PRO PV REVIVE S.A., the following are required:
- Completion and signing of the Accession Agreement.
- Completion and signing of the Obligated Producer Information Form.
- Completion and signing of the Periodic Declaration Form for the three calendar years prior to the producer’s enrollment, up to and including the last full calendar month preceding the signing of the Accession Agreement.
- Submission of all the above documents, along with legal documentation appropriate to your company’s legal form, to info@pvrevive.gr
After receiving and reviewing the above, the total amount of the declarations will be invoiced based on the terms of the Accession Agreement, and the contract will be finalized by the PRO.

What Documents Are Required to Join the Producer Responsibility Organization (PRO)?
To join PV REVIVE S.A., you must complete and sign the Accession Agreement, the Obligated Producer Information Form, and the Periodic Declaration Form covering the three calendar years preceding your enrollment, as well as the current year up to the last calendar month before signing the Accession Agreement.
Where Should the Required Documents Be Submitted?
All required documents, together with your company's legal documentation, should be submitted electronically to info@pvrevive.gr for review and processing.
Do I Need to Submit Declarations for Previous Years?
Yes. The Periodic Declaration Form must be completed and signed for the three (3) calendar years preceding the producer's enrollment, as well as for the current year up to and including the last calendar month before signing the Accession Agreement.
When Is the Enrollment Process Completed?
The enrollment process is completed after the required documentation has been received and reviewed by the Producer Responsibility Organization (PRO). Following the review, the declarations are invoiced in accordance with the Accession Agreement, and the agreement is finalized.

How Long Does the Document Review Process Take?
